These forms are available to customers that are purchasing a specific puppy.
You must complete a puppy purchase or adoption application in order for us to place a puppy on hold for payment, shipping or pick up.
The name on the application as purchaser must be the same as the persons name on credit card that payment is made from.
The information you submit will become your shipping label and must be correct.
Re-submit your application if you need to make corrections.

Primary pet purchase forms are password protected.
We have had other breeders and web sites attempt to duplicate our copyright forms.
Therefore our password protected pages are available only to our customers.
If you are in the process of purchasing a puppy the password will be sent to you once your payment is received.
Please send an e-mail with your phone number and questions along with a description of the puppy you are interested in.
Keep in mind it may take longer to receive an answer on a specific puppy with no number.
Puppy Number # can be found on the puppy picture.
