Rules for free  puppy & all mail in entry forms

Please read the rules for entering our drawing for a free puppy.

This will be a drawing for a free " Mystery Puppy "
Winner of this drawing will receive a free mystery puppy from our pet boutique that can be picked up in person at our location or shipped if shipping is needed.

Important! A question will be on the mail in entry form and it must be answered correctly in order for the entry form to be valid.
Each entry form must have the date and time it was printed from our web site.

1. You must be 18 years of age or older.
If you are not 18 years of age a parent or guardian can receive the pet for you.Children may enter the drawing if parent or guardians name is placed on the entry form.
Valid Photo I.D. of parent or guardian will be required to receive free pet if your name or child's name is drawn.

2. You must respond to our e-mail & phone message letting us know you are aware that you are the winner and confirm that you will take the pet offered within 48 hours of the date & time your name was drawn.
Double Check the e-mail address & phone numbers listed to be sure we can contact you or leave a message with friends or family.

3. You must be able to pick the pet up within 48 hours of the time that we have notified and confirmed you are the winner.

4. Winner will be responsible for pet shipping and all transportation & shipping expenses if needed.
If shipping is required you will need to pay all fees associated with pet shipping within 48 hours.

5. You must have valid photo I.D. to claim your prize.

6. Information on entry form must be hand written.

7. One entry form per envelope.

8. Limit one free pet per household and one winner per family. If you are a previous winner of our free puppy drawing you will not be eligible to win a second puppy from our pet boutique.
If you have won a free pet from our pet boutique, you will not eligible to enter drawing for another free pet.

9. Winner must be willing to accept any breed, sex or size of pet being offered. However you will not be obligated or committed to accept free pet you will have the opportunity to pass and a new winners name will be drawn.

10. No limit on the amount of entires you may submit by mail.

11. A question will be on the mail in entry form and it must be answered correctly in order for the entry form to be valid.

13. Each entry form must have the original date and time it was printed from our web site.
( Each entry form submitted by an person or family must have a different date and time on it )
Basically what we are trying to explain is don't print one entry form and make 20 copies and submit them to us all having the same date & time.

12. Color or Black & White Copies Accepted.
 
13. You need not be present at time of drawing to win!

14.  Original winner of pet must be present at time of receiving their prize it is the only person it will be released to. Pet will not be released to another person for pick up or transportation. A documented form must be signed by original winner stating they are accepting the prize before the pet is released.

15. Entry forms must be sent only by mail. No walk in entry forms accepted.

16. All entry forms must be hand written.
 After you have printed the free puppy entry form from our web site you must enter your answers and information by hand.

17. Winner understands if they are not able to receive their puppy under the time frame and conditions stated above a new winners name will be drawn.

18. Winner understand if they are not willing to accept puppy being offered for any reason a new winners name will be drawn.

Contest date is subject to change in the event of an emergency.
